2026 Community Health Grant Cycle

RTW Foundation has launched its 2026 Community Health Grant Cycle, that provides funding to organizations working to address barriers to safe, affordable access to care and advance health equity across New York City. We believe our communities are strongest when we work together, and through grant funding, collaboration, and collective action, we support local organizations driving meaningful change. Submit a letter of inquiry by August 10th.

Free Health Services in NYC Through Our Partners

Location

Montefiore's CICERO program provides confidential HIV/AIDS testing, treatment, and counseling in addition to primary care services. The African International Collaborative Center provides health resources to strengthen African immigrant communities.

AIRnyc works to improve health access, referral support, and care navigation.

RaisingHealth Partners, Mexican Coalition, and Mixteca provide community-based health services including screenings, care navigation, preventive care, and social services for immigrant communities in Brooklyn.

ACT Care Foundation delivers specialized health, nutritional, and community support services to underinsured New Yorkers.

Harlem United and New York Cares expand access to healthcare, mental health, and housing services, with Harlem United assisting people with harm reduction. Saving Mothers empowers mothers with self-advocacy resources, health equipment and health education to their providers. Karen's Club provides personalized and culturally responsive consulting to cancer patients of color.

Exodus Transitional Community and East Harlem Health Outreach Partnership (EHHOP) reduce barriers to free care by providing transportation to appointments, mental health services, and addressing social needs. Mount Sinai supports research for the care of patients with PACS. Violence Intervention Program helps Latinx survivors of domestic violence, and trains counselor advocates.

Send Chinatown Love, Think!Chinatown, and Welcome to Chinatown, support small businesses, food security, and social services for families in Chinatown. Hamilton-Madison House provides behavioral health services, youth education, and family support.

Karen's Club provides personalized and culturally responsive consulting to cancer patients of color.

The NYU Langone Health Undiagnosed Diseases Program, provides long-awaited diagnoses for medically complex patients who have gone years without answers after standard clinical evaluations.

Razom provides a wide range of medical supplies to first responders and hospitals in Ukraine.

Grameen Promotoras, a Grameen America community health program, supports underserved entrepreneurs across New York City with health education, screenings, and personalized referrals and care navigation.

The Floating Hospital provides comprehensive affordable medical care to patients, offering free transportation from shelters and safe houses across the five boroughs.

Community Health Action of Staten Island trains healthcare providers in health literacy and reduces barriers to health care.

Hamilton-Madison House

East Harlem Health Outreach Partnership (EHHOP)

"Our partnership with RTW Foundation has addressed barriers that previously prevented East Harlem residents from accessing healthcare. Providing transportation support and financial incentives has had a profound impact on our clinic, reducing our no-show rate by 20% and allowing us to reach more patients who have nowhere else to turn for care. This year we will expand this initiative to include critical dental services and other specialty referrals."

Mixteca

We aim to empower over 1,000 individuals with the education and resources they need to lead healthier lives.”

Harlem United Community AIDS Center

Harlem United facilitated 655 street-based medical visits to initiate and maintain Buprenorphine treatment, successfully helping many clients stay engaged with their care. Nearly all participants reported a reduction in drug use. Additionally, Harlem United distributed a total of 2,436 kits, including 1,279 wound care kits, 423 safer sex kits, and 734 naloxone kits, further supporting community health and safety.

Razom

Razom distributed over 50,000 First Aid kits to first responders and hospitals in Ukraine in the first 100 days of the war. 1,400 tactical medical backpacks were provided in addition to 38,000 bandages, tourniquets, and burn & wound dressings to this effort.

New Immigrant Community Empowerment

NICE provided 500 families with food packages, vaccine education and resources, registered people for vaccination appointments, and vaccinated over 5,000 people. They also distributed 10,000 meal and grocery packages through the NICE food access program.

ACT Care Foundation

African International Collaborative Center

The African International Collaborative Center supported 32 African American nurses and healthcare workers with a culturally-responsive support group, building coping skills & network of support.

Association to Benefit Children

ABC’s Children’s Mobile Crisis Team (CMCT) provides rapid mental health crisis response to children in Manhattan 24/7. Since their last interaction with CMCT, 99% of children avoided school disciplinary action and 100% avoided criminal justice involvement, 90% had not been admitted to hospital or ER, 93.3% had not spent time in an inpatient psychiatric unit, and 82% met or made progress toward their treatment goals with 28% fully meeting their goals.
